What is a labiaplasty?

Labiaplasty (labia minora reduction) reshapes the inner vaginal lips to reduce excess or elongated tissue. Many women choose this procedure for both functional relief and aesthetic improvement. Why Consider Labiaplasty?

An elongated or hypertrophic labia minora can create real challenges in daily life. Overhanging tissue often causes friction, leading to discomfort or irritation when wearing underwear, exercising, or during sexual intercourse. It can result in tugging or pain during intimacy. Concerns are often both physical and appearance-related. The most common symptoms reported by patients include:

  • Tugging or pain during intercourse

What causes labia minora hypertrophy?

The precise cause of labia minora hypertrophy is often unknown, but several factors have been proposed:

  • Congenital (present from birth)
  • Exogenous hormones
  • Chronic irritation
  • Pregnancy and childbirth
  • Local tissue laxity associated with aging


Scarring from vaginal lacerations during childbirth can also distort the natural appearance of the labia minora.

The Surgery

What to Expect from the Procedure and Recovery

Labiaplasty can be performed comfortably in the office using local numbing medication plus oral sedation, or under general anesthesia in an accredited surgical facility—whichever you prefer.

  • Sutures: Absorbable sutures are used; they dissolve on their own and do not need removal.
  • Recovery timeline:
    • 3–4 days of rest with elevation to minimize swelling.
    • You may shower starting the day after surgery; gently pat the area dry and apply antibiotic ointment to the suture lines.
    • Use a perineal lavage bottle for gentle wound hygiene for approximately 1 month postoperatively.
    • Wear loose-fitting, extra-large underwear and apply perineal ice packs (the kind commonly used after delivery) for comfort and swelling control.
    • Return to light activities within a few days; strenuous exercise is typically resumed in 4–6 weeks.
    • Sexual activity is avoided for 6 weeks to ensure proper healing.
    • For patients older than 35, Estrogen cream applied three times per week can support healing.


Most patients find downtime minimal and return to work and normal routines quickly.

Surgical Technique

There are multiple ways to perform labiaplasty, and some techniques have overlapping or confusing names, including edge resection, wedge resection, deepithelialization, W-plasty, laser labiaplasty, custom flask, fenestration, and composite reduction.The two most common approaches are:

  • Wedge resection: The middle portion of the labia minora is excised, preserving the natural “free edge” while creating a vertical scar.
  • Edge/trim resection: The overhanging excess tissue is removed along the edge, resulting in a longitudinal scar along the length of the labia minora.


When your anatomy allows, Dr. Carpenter prefers a separate, refined technique that preserves the natural labial edge on an anterior pedicle while excising a deep posterior wedge. This approach hides the scars deep within the natural crease between the labia minora and majora, leaving the visible “free edge” of the labia minora natural, unscarred, and aesthetically pleasing. The best technique is chosen based on your unique anatomy and goals during your private consultation.

Is Labiaplasty Controversial?

Labiaplasty is sometimes discussed in medical circles, but it is a well-established procedure when performed for appropriate reasons. The American College of Obstetricians and Gynecologists (ACOG) takes a cautious approach to elective female genital cosmetic surgery, emphasizing the importance of distinguishing medically indicated procedures (such as those addressing pain, functional interference, or documented symptoms) from purely cosmetic ones, and noting that high-quality long-term data are still evolving. 


In contrast, the American Society of Plastic Surgeons (ASPS) recognizes labiaplasty as the most commonly performed aesthetic genital plastic surgery, noting its ability to relieve symptoms like tugging and twisting of the labia and reporting consistently high patient satisfaction rates when performed by qualified board-certified surgeons.It is essential to understand that labiaplasty is not female genital mutilation (FGM). FGM is a non-consensual, harmful practice with no medical benefit. Labiaplasty is a consensual, elective procedure performed by trained plastic surgeons to improve form, function, comfort, confidence, and sexual satisfaction. It does not decrease vaginal sensitivity; in fact, many patients report enhanced intimacy because discomfort is eliminated.
